Care Planning Flow Chart

I need a care plan flow chart on a dementia patient living In a nursing home. the care plan flow chart should start from the referral stages from wherever the patient is coming from i.e hospital referral, G.P referral, family referral or self referral and every other multidisciplinary team that would be involved in the care plan for the client. please bare in mind that this is work is for UK(united Kingdom) university.
